Please contact our office and one of our staff will assist you in the registration process.
For your convenience, visit forms for your personal health record may be submitted on our portal prior to your first visit. You may also print and complete our new patient registration and health history forms to bring with you on your first visit.
Please note that due to practice policy, test results are excluded from the portal due to the sensitive health information they may contain. Please call our office to schedule an appointment to go over test results with your physician.
IMPORTANT: Emergency Situations
The secure message/information you send will not be reviewed by your physician in any given timeframe. If you have symptoms suggestive of a problem that you think should not wait until your appointment, please contact us. If you think the problem is urgent, call 911.
